Modern Pentathlon Australia
By applying for membership of the relevant State Association and Modern Pentathlon Australia (MPA), you acknowledge and agree to comply with these membership terms and conditions.
If you are under 18 years of age, a parent/guardian must make the following declarations on your behalf. By doing so, they will be responsible for ensuring that you comply with these membership terms and conditions at all times.
Application for Membership
I hereby apply for membership of the State Association and I hereby apply for membership of MPA.
I declare that:

I agree to be bound by, and to support the purposes of, the Constitution of the State Association, the Constitution of MPA, and the statutes and regulations of the Union Internationale de Pentathlon Moderne (UIPM), each as amended from time to time.

I agree to abide by all rules and by-laws of the State Association, MPA and the UIPM, as amended from time to time.

I agree to abide by and comply with all policies issued and amended from time to time by the State Association, MPA and the UIPM.
- I agree to pay such fees and subscriptions that are determined from time to time to apply to me.
I acknowledge that the membership year of the State Association and of MPA runs from 1 April to 31 March, and that my membership will end at 11.59pm on 31 March 2025 unless extended in the State Association or MPA’s complete and absolute discretion (as the case may be).
I understand that my application for membership is only finalised upon acceptance by the State Association and MPA (as the case may be).
Consent to use of images
I understand that at any State Association/MPA event or endorsed activity, I may be photographed and/or videotaped by an authorised or endorsed representative of the State Association/MPA. I acknowledge and agree that:

any images or likeness captured by the State Association/MPA are owned by that organisation, and may be used by them for promotional or other purposes without my further consent being necessary;

I consent to the State Association/MPA using my name, image, likeness and also my performance in any State Association/MPA event, at any time, by any form of media, to promote the event and the State Association/MPA; and
-
neither the State Association nor MPA is responsible for those images or likenesses captured by persons who are not their official or endorsed representatives.
Collection, use and disclosure of personal information
I understand that my membership details are collected by the State Association and by MPA for the purpose of creating and managing my membership and permitting me to participate in events and activities conducted by them. I understand this includes but is not limited to:

identifying participants in an event or activity

recording results

communicating with me and other participants about events and activities, and
-
assisting the State Association/MPA to improve and market their goods and services.
I consent to MPA using my personal information to register me on a national database controlled by MPA.
I understand that, by supplying my email address as part of this application, I consent to receive regular emails from the State Association and MPA in order to stay informed about events conducted by them. I further consent to receiving information (including via email) from MPA, including details of other events and special offers or promotions, unless I inform MPA otherwise.
If I apply for membership as an athlete, I consent to MPA providing my personal information (including, but not limited to, my name, age, date of birth, postal and email address) to the UIPM so that I can be eligible to participate in events under the control of the UIPM).
I understand that my personal information is being collected by the State Association and MPA in compliance with the Australian Privacy Principles and the Privacy Act 1988 (Cth) (Privacy Act) as amended from time to time, for the purpose of conducting and facilitating my membership. I understand that my membership application may be rejected if I choose not to share certain personal information.
I consent to my personal information being shared with MPA’s insurers in relation to any incident or personal illness that has reasonable and direct relation to my membership and/or any event that I enter.
Accessing and updating personal information
I understand that I may access and update my personal information (subject to the terms of the Privacy Act) by contacting the relevant person below:

I acknowledge that MPA's Privacy Policy also contains information about how I may complain about how my personal information is handled, used and disclosed.
Damage to property
Where I am responsible for and or cause any deliberate damage to property owned by MPA or the State Association, I agree to reimburse the relevant organisation for the cost of the repair of, or the replacement of its property and acknowledge that my membership may be terminated.
Child safeguarding
I understand that, for the protection of members and the general public, the State Association and MPA must enquire into the background of anyone who applies for, undertakes or remains in any work (paid or voluntary) that involves working with people under the age of 18 years. If that applies to me, I agree to undertake a Working with Children Check and to submit the results to the State Association and MPA (and I consent to this being shared between them).
